Appointments
We are happy to announce that we are currently accepting new patients. You can call our office at 912-882-6072 to schedule an appointment. Once you make an appointment at our office, we will reserve that time for you. If, for some reason, you are unable to keep your scheduled appointment time, call us with 24-hours notice to reschedule.
Patient Forms
Please arrive 15 minutes before your scheduled appointment to fill out the necessary paperwork. If you would like to print your forms and complete them before your appointment, you can.

Payment & Insurance
Payment
You should make payments when services are rendered unless you have arranged alternative arrangements. These payments include any co payments or deductibles.
Our office accepts cash, Visa, Discover, and MasterCard for your convenience.
Insurance
Our office accepts most dental insurance plans, but please familiarize yourself with your insurance plan and pay close attention to co-pays. You can rely on our accounting staff to prepare all the necessary forms for your dental benefits. However, your policy is an agreement between you and your insurance company.
We are proud to support local military families. As an in-network provider with United Concordia, we are here to provide the best possible care for our military patients.
Please know that you are responsible if your insurance benefits result in less coverage than anticipated. We base our treatment on what is best for you and will not render our services on the basis that insurance companies will pay all of our fees.
